What our nurses do

Hospital know-how, at your kitchen table

Home health nursing bridges the gap between the hospital and full recovery. Your nurse checks how you are healing, catches problems early, and teaches you and your family how to manage your condition, all under your physician's orders. Visits follow the schedule your doctor orders and taper as you get stronger.

  • ✓Start of care within 48 hours of referral
  • ✓Your physician hears from us at start of care, every recertification, and discharge
  • ✓A nurse on call around the clock, not an answering service
Wound care

Surgical incisions, pressure injuries, and diabetic or venous ulcers, cleaned, dressed, and measured so healing stays on track.

Medication management

A full review of every prescription and over-the-counter medicine, teaching on new medications, and watching for side effects and interactions.

Chronic disease care

Monitoring and teaching for heart failure, COPD, and diabetes, so you know the warning signs and can stay out of the hospital.

Coverage

Covered by Medicare, no copay
